As a health care provider, we are required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of your protected health information (“PHI”). How we use and disclose your PHI when you are our patient, and your rights regarding it, are described in our Notice of Privacy Practices. 2. Information we collect
Information you give us
When you request an appointment, ask for a consultation, complete an intake or order form, or contact us, we collect the information you choose to provide, such as your name, email address and phone number. Some forms, such as our consultation and new patient intake forms, may ask for health information. These forms are provided through secure, HIPAA-compliant form and practice-management services and are sent directly to those services rather than stored on the Website.
Information collected automatically
With your consent, we use Google Analytics to understand how visitors use the Website, for example which pages are viewed and whether a visitor clicks to call us or submits an appointment request. This information is collected through cookies and similar technologies, and it does not include the contents of any form you submit. If you do not accept analytics cookies, Google Analytics does not set cookies on your device and receives only limited, cookieless signals. We do not use analytics or tracking tools on the pages where you enter personal or health information (our appointment, consultation, order and payment-plan pages).
We do not use advertising cookies, session-recording tools or social media tracking pixels on the Website.

We do not sell your personal information, and we do not share it with third parties for their own advertising or marketing. We do not use your health information for marketing without your written authorization.
5. Service providers
We use a small number of trusted service providers to operate the Website and our practice. They may process information only on our behalf and only as needed to provide their services. Service providers that create, receive, maintain or transmit PHI for us are required to protect it as HIPAA requires.
- Online forms: Jotform, which hosts our appointment, consultation and order forms.
- Patient intake and records: ChiroTouch, our practice-management system.
- Payment plans: Cherry, which provides our “Treat Now, Pay Later” financing. Information you enter in a Cherry application goes directly to Cherry and is also governed by Cherry’s privacy policy.
- Website analytics: Google Analytics, only with your consent and never on the form pages listed above.
- Reviews: Trustindex, which displays our public Google reviews on the Website.
- Website hosting: our web hosting provider.
We may also disclose information when required by law, to protect the rights and safety of our patients, staff or others, or as otherwise permitted by HIPAA.
6. Your choices about cookies
When you first visit the Website, a banner asks whether you accept analytics cookies. Analytics cookies are set only if you click “Accept”. You can change your choice at any time with the “Manage consent” button at the bottom of each page. We honor Global Privacy Control and Do Not Track browser signals. You can also block or delete cookies in your browser settings.
7. Email, phone and text messages
Regular email and text messages are not fully secure. Please do not send detailed health information to us by email, text or through general contact links. If you need to share health information, call our office or use the secure forms we provide. If you choose to communicate with us by unencrypted email or text, you accept the risk that the information could be intercepted.
8. How we protect and keep information
We use reasonable administrative, technical and physical safeguards to protect the information we collect, including encrypted (HTTPS) connections to the Website and secure, HIPAA-compliant services for forms that may contain health information. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. We keep information only as long as needed for the purposes described above, or longer where medical record retention laws require.
9. Your rights
As our patient, you have rights regarding your PHI under HIPAA, including the right to see and get a copy of your health records, ask us to correct them, request restrictions on certain uses, request confidential communications and receive an accounting of certain disclosures. These rights and how to use them are explained in our Notice of Privacy Practices. 10. Children’s privacy
The Website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Website. A parent or legal guardian should complete forms on behalf of a minor patient.
11. Links to other websites
The Website links to other websites and services, such as YouTube, our online intake portal and our payment-plan provider. Their privacy practices are governed by their own policies, and we encourage you to review them.
12. Changes to this policy
We may update this Website Privacy Policy from time to time. When we do, we will post the updated version on this page and change the effective date above.
